The GKV supplementary contribution rate is an additional contribution rate that statutory health insurance providers charge in addition to the general contribution rate of 14.6%. Each provider sets its own supplementary rate - currently between 0.70% and 3.90%.
The supplementary contribution is split equally between employee and employer.
The supplementary contribution was introduced to cover rising healthcare costs. While the general contribution rate is set by law, health insurance providers can balance their individual costs through the supplementary contribution.
This creates competition between providers - efficient providers can offer lower contributions.
The supplementary contribution is split equally:
• 50% paid by the employee
• 50% paid by the employer
This regulation has been in effect since 2019. Previously, employees had to bear the supplementary contribution alone.
Yes, pensioners also pay the supplementary contribution. Here, the pension insurance takes over half of the supplementary contribution, and the pensioner bears the other half.
However, for company pensions, pensioners must pay the supplementary contribution completely themselves.
The supplementary contribution is calculated as follows:
Gross income × Supplementary contribution rate = Supplementary contribution
Example with 3,500€ gross and 2.5% supplementary contribution:
3,500€ × 2.5% = 87.50€/month

Yes, the contribution assessment ceiling in 2026 is 5,812.50€ monthly (69,750€ annually).
This means: If you earn more, you pay a maximum contribution up to this limit. Higher incomes are not subject to further charges.
Switching is simple and straightforward:
